Two things:

First, when measuring and cutting the top and bottom of your cushion you have to make sure its a perfect square, or perfect rectangle.  The only way I know of to do this is to mark the measurements on your fabric and then measure each piece diagonally BOTH directions.  This will ensure you have two true measurements and your top and bottom match.  If they're off even a tiny bit, it will accumulate as you sew and make a funky corner.

Next, after you've attached your box strip to the first piece, take the corner and fold the box strip and notch at the opposite side of the box strip itself, so that you are going straight up from the sewn corner to the not yet sewn corner.   Do that on all four sides.  Then, match the corners from the 2nd piece to those notches and pin.  Then sew.

Also, when you go around the corner, lift up the presser foot and make a nice 90 degree corner.

Hope that helps.
